Painful menstruation, medically known as dysmenorrhea, is one of the most common health problems among women. It is a condition in which menstruation is accompanied by intense, cramping pain in the lower abdomen, sometimes radiating to the lower back, legs, or pelvis. Many women
consider this pain “normal” because they have experienced it since their teenage years and believe it is simply a natural part of menstruation. The truth, however, is that severe pain during menstruation is a signal that the body needs something—most often relaxation, balance, and a change in lifestyle.

One of the main causes of painful menstruation is increased tension in the abdomen, pelvis, and pelvic floor. These structures are closely interconnected, and when any one of them is overloaded or not functioning properly, it affects the entire menstrual cycle.
Pain can also result from poor posture, increased stress, improper breathing, a sedentary lifestyle, or weakness in the deep stabilizing muscles. Menstrual pain is also influenced by hormonal changes, inflammation, or conditions such as endometriosis. Whatever the reason, it’s important to know that this pain isn’t normal and you don’t have to endure it every month.

You can learn more about endometriosis in our article: Endometriosis: Symptoms, Diagnosis, and How Intimate Physical Therapy Can Help

Intimate physical therapy offers a holistic approach that treats a woman’s body as an interconnected system. The goal of therapy is not merely to suppress symptoms, but primarily to improve the function of the pelvic muscles, abdominal wall, and pelvic floor, which often contribute to the development of menstrual pain. A very important part of the therapy is releasing overloaded structures. A tense pelvic floor, a tight lower abdomen, or blockages in the lower back and lumbar spine can significantly affect blood flow and organ mobility. When the tissues in this area are released, the menstrual cycle often becomes smoother and less painful.

Endometriosis is a chronic gynecological condition that affects approximately one in ten women. It is a condition in which tissue similar to the lining of the uterus (endometrium) is found outside the uterus—most commonly on the ovaries, in the
fallopian tubes, on the bladder, in the pelvic area, or even on the intestines. This tissue reacts to hormonal changes in the body and can cause recurrent bleeding, inflammation, and the formation of adhesions. The exact cause of endometriosis is not yet clear, but it is believed to involve a combination of genetic factors, immune disorders, hormonal imbalances, and chronic stress.

Diagnosing endometriosis is not easy and often takes years. The process begins with a medical history and a gynecological exam. An ultrasound can reveal typical cysts—endometriomas. In more complex cases, doctors use magnetic resonance imaging,

which can pinpoint the location of the lesions more accurately. However, a definitive diagnosis can only be confirmed by laparoscopy—a minimally invasive procedure that allows the lesions to be seen directly and diagnosed with certainty.

How Experiencing Emotions Affects the Pelvic floor

Mental and emotional health is a key factor that influences our overall health. In today’s fast-paced and often stressful world, we’re increasingly faced with demands on our performance, which can have a negative impact not only on our minds but also on our bodies. One area that can be indirectly affected by our emotional state is the condition of the pelvic floor. What do the nervous system and the pelvic floor have in common, and how does this manifest in our daily lives and relationships?

Stress, Emotions, and Relationships

A fast-paced, performance-oriented lifestyle and the constant pressure to achieve our goals can have a devastating impact on our emotional health. This constant pressure can cause our bodies to remain in “fight-or-flight” mode over the long term, leaving no time for rest.

The sympathetic and parasympathetic nervous systems are two components of the autonomic nervous system, which controls the body’s automatic functions. The sympathetic nervous system is active in stressful situations when the body needs to respond quickly to external stimuli, which is vital for survival. Conversely, the parasympathetic nervous system is responsible for relaxation and regeneration—it allows the body to rest and restore energy. However, if we frequently experience stress, our body remains constantly in sympathetic mode, which can have a negative impact on various bodily functions. That is why the mutual regulation or balance of sympathetic and parasympathetic nerve impulses is so important, as both are essential for every person’s survival and health. Intimate physical therapist Katka Hanuliaková explains this topic in more detail: “We live in a fast-paced, performance-driven era, and many people have forgotten how to regulate themselves, find peace, and relax.”

Our goal is not to be in a constant state of meditation; we also need to know how to activate the sympathetic nervous system and be able to cope with stressful moments or periods. However, many people remain in the sympathetic nervous system’s “fight-or-flight” mode for too long—or even permanently—and that’s when problems arise. This is reflected in a person’s life through the functioning of their relationships and emotions. It has been proven that in the rush and drive for performance, relationships take a back seat because an activated sympathetic nervous system prevents us from experiencing deeper emotions. Only when we are at peace and our nervous system is regulated do we begin to “process” what we have experienced, and once the nervous system is regulated, space is created for relational bonds and for deepening intimacy. A person who is focused solely on performance disconnects not only from others but also from themselves. They cease to perceive themselves and their basic needs.”

Don’t ignore yourself

One of our basic needs is the need to empty our bowels—urination and defecation. In our society, however, this is not considered “acceptable,” and because of stress and responsibilities, we’ve become accustomed to outright ignoring our own defecation reflex. When the impulse arises that our large intestine is full, we feel the urge to have a bowel movement, and that’s when we really should go. But we aren’t accustomed to having a bowel movement in public restrooms or at work. Many people save it for the safe environment of their own home, where they can relax. By doing so, we disrupt the functioning of our excretory system, and it also amounts to ignoring our own bodies and basic needs, which increases tension in the pelvic floor. And high tension in the pelvic floor causes a whole range of other health problems. This learned habit of ignoring our own needs can even lead to a disconnection from ourselves. We become insensitive to our own bodies and only react to stimuli that are already painful or alarming.

“A person who is not connected to themselves will find it very difficult to connect with another person or their partner. Such a person may have a sex life and engage in sexual intercourse, but it is not usually on a deep level. Let’s try to be more respectful of our basic needs, be more attuned to our own experiences, as well as our partner’s experiences and the needs of the relationship. “The foundation for this is the ability to slow down, relax, and regulate one’s nervous system,” Katka Hanuliakováadds, furtherelaborating on the issue of emotional health.

From an anatomical perspective, we can explain the impact of emotions on our body’s functioning in a fairly simple way. Our emotions are accurately interpreted by the central nervous system, which continuously receives sensory input from the environment. The nervous system responds to these sensations and sends signals to the skeletal and striated muscles. At the same time, the autonomic nervous system regulates the function of individual internal organs. The hypothalamus and the limbic system play the most significant roles in communication with the internal organs. The hypothalamus is a part of the brain that is highly sensitive to the emotions we experience. If these emotions become too overwhelming, the limbic system alters the activity and function of individual internal organs.

If we wanted to compare this anatomical functioning of the nervous system and the impact of experiencing emotions in today’s fast-paced, performance-driven world to something, we could describe it as driving a car. Imagine you’re pulling out of your driveway; at first, you drive slowly through the narrow streets, keeping to the 30 km/h speed limit.

At this speed, you’re relaxed; you even have time to take in your surroundings—who’s walking on the sidewalk, what kinds of trees are growing along the road. But then you get onto a faster road and, via the on-ramp, reach the highway in a very short time, where the speed limit is ninety plus, and in a very short span of time you accelerate your car to high speed. Our nervous system works the same way. For the first few seconds and minutes, we need to focus fully on the drive and “grip the steering wheel a little tighter”; we experience varying degrees of tension, no longer perceive our immediate surroundings, focusing only on the road and the car’s speed.

What happens later? However, if we drive on the highway for a longer period of time, we adapt, relax, stop perceiving the stress, and instead start communicating with the other people in the car and even notice the surrounding nature along the way. But after a while, the situation changes again, and we have to slow down from our high speed on the highway because we’re entering a village. And this sudden drop in speed, after driving so fast, seems unbearably slow; it makes us feel uneasy and irritable. And this is how our nervous system works as well—we can very quickly ramp up to a high-performance state, get stressed out, and remain in this state for quite a long time. But after such a period of high performance, we need to be able to regulate and calm ourselves in order to maintain our overall health. That’s why at Intimfyzio, we also help clients practice and train their regulatory abilities.

Proper breathing—especially diaphragmatic breathing—is a fundamental pillar of our body’s optimal functioning. Its importance extends beyond physical health; it also affects our hormonal balance and fertility. Improper breathing habits and poor posture can lead to excessive tension in the pelvic area. According to Katarína Hanuliaková from our Intimfyzio center, breathing is the key to pelvic floor health, hormonal balance, and fertility. By practicing proper breathing and improving your posture with the help of physical therapy, you can support your ability to conceive.

Proper breathing isn’t just about inhaling and exhaling—it’s a technique that should involve using the diaphragm, the main breathing muscle located between the chest and the abdominal cavity.

Diaphragmatic breathing, or so-called “breathing with the diaphragm,” is a frequently discussed concept. It is supposed to be the primary form of breathing we use in our daily lives. When we engage in more intense physical activity during the day, the accessory breathing muscles join the diaphragm to help ensure adequate oxygenation of the body. During inhalation, the diaphragm moves downward and expands the lower ribs. This downward movement of the diaphragm creates pressure on the organs of the abdominal cavity and pelvis.

What is diaphragmatic breathing?

One of the basic functions of the pelvic floor is to support and regulate intra-abdominal pressure. As soon as the diaphragm descends during inhalation, the abdominal wall expands, pressure is exerted into the pelvis, and our pelvic floor gently and naturally descends. When we exhale, the exact opposite happens—the diaphragm contracts and rises, and the pelvic floor responds by rising as well, drawing inward and performing what is known as a concentric contraction. This process flows continuously within our bodies in this way, without us even realizing it.

Our normal breathing throughout the day should be calm and sufficiently deep. However, it is important to be able to adapt our breathing to various stressful situations. The same goes for our pelvic floor. For example, when we go for a run, the auxiliary respiratory muscles must assist diaphragmatic breathing; our breathing and heart rate accelerate, and the intensity of our breathing increases. After finishing our physical activity, it’s necessary to “breathe out completely.” Subsequently, our breathing slows down, and calm, diaphragmatic breathing resumes. The pelvic floor always responds to changes in the intensity and depth of breathing.

Poor Posture as an Obstacle to Proper Breathing

However, if we don’t have ideal posture—which is common these days—our rib cage may be misaligned, triggering a chain reaction of problems all the way down to the pelvic floor. Physical therapist Katka Hanuliaková explains the connection between poor posture, improper breathing, and pelvic floor function in more detail:“In our physical therapy practice, we often encounter what’s known as ‘open scissors syndrome,’ where we see that our clients’ ribs protrude forward. In this condition, when a client tries to breathe using the diaphragm, their pelvic floor is not positioned directly beneath the chest and therefore does not function properly. From a physical therapy perspective, the ideal state is when the chest is positioned directly above the pelvis, and with each inhalation and exhalation, the diaphragm properly influences the activity of the pelvic floor.

So, in order to breathe properly, it’s important to maintain good posture and center the entire rib cage—including the diaphragm—above the pelvis. This ensures that we can utilize the full capacity of the diaphragm and that our pelvic floor will function properly as well.”

Shallow breathing, poor digestion, and reduced fertility

With every proper inhalation, our abdominal and pelvic organs are massaged. It is important to realize that this constant movement of the diaphragm also stimulates our endocrine and reproductive glands. Diaphragmatic breathing thus also affects the function of the ovaries and testicles themselves. Once we’ve learned to breathe shallowly, as if using only the upper part of the body, the stimulation of these glands is limited. The motility of the organs is also limited, which very often manifests as digestive disturbances. With insufficient breathing, the diaphragm does not descend, the intestines are not massaged, and the entire peristaltic process is slowed down, as is the act of defecation itself. Undigested stool, which should have been naturally eliminated from the body as waste, is instead retained, pressing on the pelvic floor muscles and creating increased tension in this area. As we already know, prolonged increased tension in the pelvic floor causes a number of health problems and also negatively affects couples’ fertility and ability to conceive.

“As part of an individual assessment of a person or couple who have been trying to conceive for an extended period of time, our pelvic floor diagnostics also focus on evaluating our clients’ breathing, overall posture, movement patterns, and lifestyle. When helping couples conceive, it is often precisely these identified deficiencies that are directly linked to their reduced fertility,” explains Katka Hanuliaková, an intimate physical therapist, regarding her work with couples on their journey to having their long-awaited baby.

The journey toward having a baby can unexpectedly involve a long period of waiting and uncertainty. According to reproductive health experts, infertility is becoming an increasingly common problem for couples, often including young people. Even though the results of gynecological or andrological examinations are within normal limits, many couples suddenly find themselves facing a diagnosis of functional infertility—that is, infertility without a clear cause. It is precisely in these cases that physical therapy focused on the pelvic floor and overall body relaxation can be very helpful.

Physical Therapy and Its Impact on Fertility

Physical therapy is often associated with rehabilitation following injuries or surgeries, but it is becoming increasingly clear that it can also play an important role in treating functional infertility. Here at IntimFyzio, we work with clients who have various pelvic floor issues, including fertility problems. Functional infertility can be caused by improper pelvic floor muscle tone or breathing problems, which lead to insufficient blood flow and oxygenation of the reproductive organs, thereby reducing their functionality.

Katka Hanuliaková, an expert in intimate physical therapy, explains: “Many couples who come to us have already undergone a wide range of tests, but the results are within normal limits. However, it’s important to know that if the pelvic floor muscles aren’t functioning properly, they can literally block the natural bodily processes that are crucial for conception. So even if everything is ‘fine,’ the problem may actually lie inimproper muscle tone or incorrect breathing.”

How can pelvic floor problems affect fertility?

The pelvic floor is a group of muscles that plays a key role in supporting the organs in the pelvic region and ensuring their proper function. If the pelvic floor muscles are too tense or, conversely, too weak, this can cause problems with circulation and oxygen supply, which can impair the function of the reproductive organs. Proper functioning of the reproductive organs is also significantly influenced by quality breathing. If breathing is shallow and irregular, the organs in the pelvic region receive less oxygen, which negatively affects their ability to perform all their functions. Proper breathing techniques contribute to the nourishment of the reproductive organs and to the harmonization of the hormonal systems in both women and men.

Physical Therapy Approaches to Functional Infertility

Physical therapy techniques aimed at improving fertility include working on proper breathing, as well as exercises to relax and strengthen the pelvic floor. At our center, we view intimate physical therapy as part of holistic care for the body, mind, and emotions. Our clients often feel a sense of relaxation after just a few sessions, as they begin to be more attuned to their bodies and learn to release excess tension and stress. “This can have a positive impact on their fertility, as the body functions more harmoniously as a whole,” says Katka Hanuliaková. With regular physical therapy, it is possible to improve oxygenation and blood flow to the pelvic region, which can increase the chances of conception.

When should you visit us?

If you’ve been trying to conceive for a long time and all your test results are normal, seeing a physical therapist who specializes in the pelvic floor may be a good step to take. Physical therapy offers a different perspective on fertility and can help identify and alleviate problems that conventional methods may miss. Katka Hanuliaková concludes by emphasizing: “It’s often the case that a physical therapy approach is the missing link that helps a couple achieve their dream of starting a family. Every body is different and has its own specific needs—our role is to help the body find harmony so that it is ready and able to conceive.” With regular physical therapy, it is possible to improve oxygenation and blood flow to the pelvic area, which can increase the chances of getting pregnant.
